Revelation of Myocardial Etiologies through Cardiac Troponin Fragment Composition

undefinedSince 1994, Hytest has systematically studied the biochemical and immunochemical properties of cardiac troponins and has developed comprehensive antibody selection strategies for cTnI and cTnT assay development, backed by a strong scientific foundation. To support our scientific knowhow, Hytest scientists in collaboration with the groups of Dr. Zhenlu Zhang (Wuhan Asia Heart Hospital), Fred Apple (Hennepin Healthcare/Hennepin County Medical Center), and Yi Zhang (Mindray Bio-Medical Electronics Co.) have done another extensive research study, compiled in the latest article Characterization of Cardiac Troponin Fragment Composition Reveals Potential for Differentiating Etiologies of Myocardial Injurypublished in March 2025 in the Clinical Chemistry Journal.  

The goal in this study was to compare the composition of cardiac troponin in patients with acute and chronic heart conditions. Plasma samples were collected from different patients with acute myocardial infarction and cardiac surgeries as well as with chronic heart failure and chronic cardiomyopathies. Major cTn forms in the circulation and their ratios were analyzed using various anti-cTn immunoassays. Patients with type I myocardial infarction and after cardiac surgery showed a high ratio of the ITC complex that comprised long cTnT (long cTnT-ITC) to cTnI and low ratios of cTnT to cTnI that rapidly decreased in time after the revascularization or percutaneous intervention. In chronic disease, cTn composition showed a stable and low ratio of long cTnT-ITC to cTnI and high ratio of cTnT to cTnI. These findings indicate the difference in kinetics of different forms of cardiac troponin and may contribute to the differentiation between acute injury and chronic cardiac diseases.
